The Trust, in collaboration with Cigna, offers eligible former players Supplemental Mental Health Benefit coverage for certain outpatient mental health services.
The Supplemental Mental Health Benefit FAQs
Any former player with 2 or more Credited Seasons who is enrolled in The Trust is eligible for the Supplemental Mental Health Benefit (SMHB). Spouses/Dependents are NOT eligible to utilize the SMHB.
A former player is enrolled in The Trust when he has completed The Trust Enrollment Process, which includes the following:
- Submitting signed Trust Participation and Consent Form(s) to The Trust
- Completing The Trust Intake with a Trust Program Manager/Specialist
The enrolled former player and the former player’s provider will receive confirmation of the former player’s enrollment in the Supplemental Mental Health Benefit within 3-5 business days from the day on which the Program Manager/Specialist submits the referral to Cigna.
The Supplemental Mental Health Benefit (SMHB) covers the following:
- Outpatient Mental Health Counseling*
- Outpatient psychiatry appointments
- Intensive Outpatient Programs (substance abuse and/or mental health)
- Partial Hospitalization Programs (substance abuse and/or mental health)
- Telemedicine access for outpatient counseling and psychiatric visits
*Outpatient mental health counseling will be covered under the SMHB after a former player EAP benefits have been exhausted.
The Supplemental Mental Health Benefit does not cover the following:
- Medication
- Neurological testing or Neuropsychiatry services
- Laboratory services related to mental health or substance abuse treatment
- Housing expenses related to mental health or substance abuse treatment
- Travel expenses related to mental health or substance abuse treatment
- Marriage and Family Therapy
The Supplemental Mental Health Benefit (SMHB) covers only the specific services outlined above, and is NOT a replacement for health insurance. Because the SMHB is not insurance, former players will not be issued an ID card.
There is no cost associated with using the Trust Supplemental Mental Health Benefit.
The Trust will cover an annual maximum benefit of $10,000 to be utilized by the former player for the listed mental health services between March 1st and February 28th. The $10,000 annual maximum renews annually on March 1st. Please note: The $10,000 annual maximum is per Former Player (for any combination of covered services provided by all providers), not per provider.
The Trust Supplemental Mental Health Benefit will provide 100% financial coverage for eligible outpatient mental health services (as outlined above). There are no out of pocket costs to the enrolled former player, provided that one of the following criteria is met:
- Treatment Providers are within the Cigna network; OR
- Treatment providers have signed a Letter of Agreement with Cigna to bill for services provided to the former player
The Trust Supplemental Mental Health Benefit will provide secondary financial coverage for eligible outpatient mental health/substance abuse services not covered by the former player’s insurance. Co-pays and deductibles not covered by the former player’s insurance can be billed to the SMHB, provided the treatment provider is within the former player’s insurance network.
The SMHB will pay for eligible services received after the former player has been enrolled in the SMHB.
A former player may enroll in the Supplemental Mental Health Benefit ONLY via either of the following contacts:
- Former Player’s assigned Trust Program Manager/Specialist
- Trust Medical Partner, after completion of The Trust Medical Assessment
- Uninsured former players should contact their assigned Program Manager/Specialist for assistance in locating a provider.
- Insured former players should contact their insurance provider to obtain a list of providers in their area. Once a provider is identified, the former player should contact their Trust Program Manager/Specialist to complete the Supplemental Mental Health Benefit Enrollment Process.
- If a former player completes The Trust Medical Assessment administered by a Trust Medical Partner, the Trust Medical Partner will assist the former player in identifying a provider in the former player’s home community.
Former players interested in changing to a different provider should contact their Program Manager/Specialist as soon as possible prior to making a change.
